Open House to review Health Services Plan

Community Health Open House to review health services plan
Tuesday, November 4, 11:30 AM – 12:30 PM
Purple/Red Skyway at HCMC

How does Hennepin County Medical Center (HCMC) work to advance health equity and address the health needs of its community? Information about the programs and partnerships created to address community health needs will be available for review at a Community Health Open House on Tuesday, November 4 from 11:30 am to 12:30 pm in the Purple/Red Skyway at HCMC’s downtown campus.

Patients, employees, visitors, and community members are invited to attend the Open House and talk to HCMC staff about community health planning and initiatives for the coming year. Additionally, HCMC staff will be on hand to hear what the community has to say about HCMC’s 2015 Health Services Plan. The Health Services Plan describes the health needs of the community and HCMC’s role in meeting those needs. This year’s report details how HCMC will work to advance health equity while supporting improvement in maternal and child health; mental health; and nutrition, obesity and physical activity.

The Health Services Plan is available online at http://www.hcmc.org/about/community-involvement, or anyone can request a copy by calling 612.873.9516.
