Community Health Open House to review HCMC’s Health Services Plan and Community Needs Assessment

How does Hennepin County Medical Center (HCMC) address the health care needs of its community? The results of a new community health needs assessment and information about the programs and partnerships created to address those needs will be available for review at a Community Health Open House on Tuesday, October 22 from 4:30 to 6:30 PM at Whittier Clinic, 2810 Nicollet Ave. in Minneapolis.

The public is invited to attend the Open House and talk to HCMC staff about community health planning and initiatives for the coming year.  HCMC staff will be on hand to hear what the community has to say about HCMC’s 2014 Health Services Plan.

“It’s very important to us to understand how we can address any gaps in our planning,” explains Libby Jensen, Outreach Program Coordinator, Public Policy and Strategy at HCMC. “We certainly have data to support our plans, but plans are about people. Nothing can replace the value of hearing from and partnering with community members.”

The Health Services Plan and Community Health Needs Assessment describe the health needs of the community and HCMC’s role in meeting those needs. The draft report details how HCMC will provide services and partner with the community to support improvement in maternal and child health; mental health; and nutrition, obesity and physical activities.

Hennepin County Medical Center is a nationally recognized Level I Adult Trauma Center and Level 1 Pediatric Trauma Center with the largest emergency department in Minnesota. The comprehensive academic medical center and public teaching hospital includes a 462-bed acute care hospital and primary care and specialty clinics located in downtown Minneapolis and St. Anthony, Brooklyn Center, Brooklyn Park, and Richfield; and in Minneapolis in the Whittier Neighborhood and on East Lake Street. HCMC also has a retail clinic in Bloomington, an employee clinic in the Hennepin County Government Center, and a Neurology and Specialty Clinic in the 212 Medical Center in Chaska.
